Greatly wounded by a Thal bombardment of his laboratory;
Lost his left arm, entire lower body & eyes, had an eyestalk-bulb in his forehead to see;
Confined to a mobile life support system attached to a wheelchair
Created the Daleks, in an attempt to end the war, they tried to exterminate him
Saved from death by Dalek Caan during the first year of the Time War
Used his own flesh to create a "new empire" of Daleks
Tried to create a reality bomb, stopped by the Doctor & his allies
Buried alive when the roof of the building he was in collapsed on him
